Acetabular labral tears are common and believed to lead potential damages in hip articular cartilage. Impaired joint sealing after labral tears may cause rapid fluid exudation and result in increased cartilage stresses. Although biomechanical changes are critical to understand the effect of torn labrum on osteoarthritis development, no studies have investigated realistic cartilage deformational changes after labrum damages. In this study, we aimed to measure alterations in cartilage deformation before and after labral tears under a realistic cyclic loading condition. We tested 8 pig hips in normal standing position. We created a cyclic loading with a peak pressure of 150 kgf (1.5 body weight) and loading frequency of 0.07Hz (15 second duration). Prepared samples were scanned in a CT scanner while the samples were continuously loaded. A hip sample was tested when the labrum was intact in the 1st day and when the labrum was removed in the 2nd day, and the differences were observed. We found that the area of abnormally large deformation was significantly increased in labrum-torn hips. Labrum tears seemed to create elevated deformations in hip articular cartilage and may result in rapid dehydration of hip cartilages which is detrimental to articular cartilage and result in osteoarthritis.
